After filling CSC form must signed
CSC online application form and then upload on university portal (Agency Number 10611 )
Highest Degree Certificate (Notarized copy) If you are a university student then provide Hope Certificate
Transcripts of Highest Education (Notarized copy)
A personal statement ( Submit a personal statement indicating your past academic background and a study plan or research proposal , Not less than 800 words in English)
Two Recommendation Letters
Curriculum vitae (The applicants submit a CV introducing your personal information, educational background, work experience, research work, publications, honors, and other information )
Passport Copy
Upload original Physical Examination Form with Blood Test reports, Electrocardiograph and X-Ray Reports (Click Here to Download)
Certificate of No Criminal Record (Police clearance Certificates)
English Proficiency Certificate (IELTS is not Mandatory)
