  1. CSC Online Application Form   (Agency no for B category 10451)
  2. Highest Degree Certificate (Notarized copy) If you are a university student then provide Hope Certificate
  3.  Transcripts of Highest Education (Notarized copy)
  4. Personal Statement not less then 500 words
  5. Two Recommendation Letters By Professors and Associate Professors (Not from Assistant professors and Lecturer)
  6. Passport Copy
  7. Recent passport size photo with white background size of 35mm×45mm
  8. Physical Examination Form Photocopy
  9. English Proficiency Certificate (Ielts is not Mandatory)
  10. Academic articles or achievements (if applicable)
