Send two sets of documents to university

1. CSC Online Application Form   (Agency no for B category 10338)
2. Online University form Zhejiang University of Science and Technology
3. Highest Degree Certificate (Notarized copy)
4.  Transcripts of Highest Education (Notarized copy)
5. Study Plan or Research Proposal
6. Two Recommendation Letters By Professors and Associate Professors (Not from Assistant professors and Lecturer)
7. Passport Copy with my signature
8. Physical Examination Form Photocopy
9. English Proficiency Certificate (Ielts is not Mandatory)
10. HSK certificate if any (its optional)
11. Acceptance letter (if available)
