6. Physical Examination Form (How to Prepare Physical Examination Form Click Here)
7. Two Recommendation Letters By Professors and Associate Professors (Not from Assistant professors and Lecturer) (issued after January 1, 2021) (How to Prepare Recommendation Letters Click Here)

9. HSK Certificate for Chinese taught Program
10. No criminal record certificate (issuance date must be after January 15, 2021)
